SAMHSA Speaker Request Form
To invite the Assistant Secretary for Mental Health and Substance Use or other SAMHSA Official to speak at your event, please complete the online form by filling out SAMHSA Speaker Request Form. Please be sure to provide full, accurate and specific information. We ask that you submit this form preferably 12-weeks prior to the event date.

Visitor Parking
Visitor parking permits are distributed on a first-come, first-served basis. Visitors can also pay to park in the public garage at 5625 Fishers Lane. Parking requests must be made at least 24 hours in advance. Email requests to parking@hhs.gov and include the following information:
- Date parking is needed
- Time parking is needed
- Visitor name
- Visitor email address
- Vehicle tag number and state
- Purpose of visit
- Escort’s name and phone number
