Information is provided for applicants on responding to the evaluation criteria in the Data Collection and Performance Assessment section of the Project Narrative.
Data Collection, Data Management, and Performance Monitoring
Consider these questions:
- What data collection software/instruments will we use?
- How often will we collect data?
- What processes will we implement to ensure accurate, timely data collection and recording?
- Who will be responsible for collecting and recording data?
- How will we consider the language, norms, and values of the population(s) of focus when developing data collection methods?
- How will we ensure confidentiality and obtain informed consent (if applicable)?
- What data collection procedures will be put in place for partners and/or sub-recipients (if applicable)?
- What data protection policies and procedures will we put in place, including information about storage, retention, and access?
- How frequently will we review and monitor performance data?
- Who will be responsible for conducting data analysis and evaluation and completing reports?
- How will we report data to staff, stakeholders, SAMHSA, an Advisory Board, and other relevant project partners?
These examples may provide ideas about how to present this information in your Project Narrative. These are only suggestions. You are not required to use a table format.

Using Data to Enhance the Project/Quality Improvement (QI)
Questions to consider:
- What QI model will we use?1
- How will we use the QI process to track progress?
- Who will be responsible for overseeing QI processes?
- If changes to project implementation and/or project management are needed, how will we approach that?
- What decision-making processes will be used?
- When will decisions be made concerning project improvement and who will make those decisions?
- What are the thresholds for determining what changes need to be made?
- What role will the Advisory Board have in the QI process?
- How will the changes be communicated to staff and/or partners/sub-recipients?
